Project overview

 
Diabetes is a condition affecting a large number of Albertans, with studies showing up to a quarter of the population living with diabetes or prediabetes.  The administrative overhead and time required to actively support such a large number of patients is challenging for the typical primary care clinic. The Provincial PCN Diabetes Management Awareness Campaign Initiative, a partnership between Boehringer Ingelheim, Brightsquid, and Health Cities, aims to alleviate administrative challenges. The initiative will utilize secure messaging to effectively and efficiently identify and engage prediabetic and diabetic patients, prompting them to proactively visit their primary care provider or CDM program. 

Call for Participation

Health Cities is seeking primary care network team members who are familiar with chronic disease management to provide feedback and guidance on campaign content. Interested parties will participate in virtual working groups that will support the development of materials associated to the identification of appropriate patients, develop a patient screening questionnaire, and design success criteria to evaluate the success of the prediabetic/diabetic patient awareness and engagement campaign. 

Commitment:  

Participation in the project will include a time commitment of approximately four hours over twelve-week period throughout December 2022 to February 2023, which would include attendance at: 

  • One or two hour-long virtual working group sessions to provide your input and feedback on campaign content and validate the final product 
  • One virtual working group session to review lessons learned and initiative outcomes 

Intended outcomes of the working groups include:   

  • Engaging with secure messaging partner to design and develop the patient awareness campaigns  
  • Identifying two or three PCNs to implement the secure messaging tool to test/trial the awareness campaign  
  • Evaluating campaign success to inform future scale and spread
