You're Invited: Health Cities ALC Engagement Sessions

As part of the Alternate Level of Care (ALC) transitional bed pilot initiative, Health Cities is inviting ALC Care and Service Providers to a focused, 60-minute engagement session to help inform how Alberta can better support individuals designated as ALC and modernize care transitions across health and community systems.

About the Initiative

The ALSS | ALC Initiative, led by Health Cities in partnership with the Government of Alberta, Ministry of Assisted Living and Social Services (ALSS), addresses a critical challenge: people who no longer need hospital-level care but remain in hospital due to limited alternatives.

Working with provincial agencies, service providers, innovators, and communities, we support the co-development of transitional and longer-term models of care that can scale.

Purpose of this engagement

We are conducting a multi-phase engagement over the next month and are seeking your insights as part of the ALC ecosystem.

Our goal

  • Understand your operational realities, and current capacity constraints
  • Explore the types of ALC patients your teams are able (or unable) to support, and why
  • Identify opportunities for transitional, short-stay, or innovative models of care
  • Hear your ideas about what could be possible with the right partnerships, flexibility, or funding
  • Outline how to contribute feedback and submit proposals for trials that validate models of care

Who should attend

Senior leaders, operational leads, and/or clinical leads from Continuing Care. Please share this opportunity with colleagues in placement, clinical care, operations, or innovation who bring valuable perspectives.

How to participate

You may attend the session that best fits your role. Please register via the registration link for the session you plan to attend. You may register for more than one session if you wish.

Format & Access

The sessions will be held over Zoom (60 minutes), and each session will be recorded. A transcript or summary is available upon request.

Privacy

This is a private event for professionals in the industry and will not be shared or distributed publicly. Data will be used for project reporting and initiative planning.

Session Options

Each session is 1 hour. You may choose the session that best fits your organizations role. We have identified the groups below. For more information on each group, and to register for a session, click the group name. 

For the purposes of these engagement sessions, we have defined this group to include Type A, Type B, and Secure spaces. These facilities are governed under the Continuing Care Act and often run by the same organizations. These organizations follow similar referral, admission, and funding pathways.

For the purposes of these engagement sessions, we have defined this group to include hospice care, home care, and restorative or rehabilitative programs. These services support patients outside of acute care and help stabilize, recover, or transition individuals back to the community.

For the purposes of these engagement sessions, we have defined this group to include supportive living and retirement facilities that are not contracted by AHS. These sites are regulated only under accommodation standards and operate under different business models.

For the purposes of these engagement sessions, we have defined this group to include shelters, housing agencies, Indigenous-led groups, FCSS, and outreach services. These organizations serve patients who often fall outside of standard care models.
